Your First Visit
Welcome to Shoal Creek Family Medicine and Allergy. We appreciate the opportunity to be part of your healthcare team, and we look forward to meeting you.

At your first appointment, a detailed medical history will be taken to carefully evaluate your condition. Before your appointment, there are several important steps you can take to make this process as easy as possible:

1. Arrive to your first appointment at least 15 minutes ahead of time
   
2. Bring any current medications with you to your appointment
   
3. Bring your completed Patient Health History Form and Patient Registration Form (download below) with you to your appointment
   
4. Ask your previous primary care physician to forward copies of your medical records to us, including recent lab reports, electrocardiograms and chest X-rays.
   
5. Bring all your insurance cards and a copy of your picture ID and applicable co-payments. If you do not have health insurance, please note that your entire balance will be expected at the time of service. We encourage you to call the representatives in our billing department at 816-781-4244 extension 224 if you’d like to discuss questions about our financial policies.
   

We expect payment in full at the time of service, unless you provide complete and accurate insurance information so we can file for reimbursement. All applicable co-payments, co-insurance and deductibles are due at the time of service.
